What Is The NDIS?
The National Disability Insurance Scheme is referred to as the NDIS. It is a relatively new funding pathway for people living with a permanent disability that impacts their ability to function and participate in everyday life activities.
If deemed eligible, you will be provided an individualised NDIS plan and enter the scheme as an NDIS participant. Your NDIS plan will outline the supports you require to meet your needs and achieve your goals within a 12 or 24 month period. Your NDIS plan will also include allocated funding for these supports.

Refer to the NDIS website for more information.

Working within the NDIS funding body, United Health accepts referrals from plan-managed and self-managed participants.

How to access United Health services

If you are plan-managed, let your support coordinator or plan manager know that you would like to access our services. They will get in touch with us to discuss the support services you wish to access and initiate the referral process. One of our therapists will then contact you to set up an initial appointment.

If you are self-managing your NDIS plan or you do not have a plan manager or support coordinator, you can contact us directly by phone or email, or alternatively by filling out the form on the contact us page. One of our team members will then get in touch with you to discuss the referral process and get you on your way to accessing services.
